- Primary diagnosis and relevant medical history
- Current footwear issues or clinical concerns
- Mobility status and ambulation aids used
- Funding pathway (NDIS, DVA, EnableNSW, Private)
- Photos of feet/footwear (helpful but optional)
What We Provide Back
- Appointment confirmation: Email when patient is booked
- Assessment summary: Key findings from initial consultation
- Treatment plan: What we recommend and why
- Supply notification: When footwear/orthoses are fitted
- Follow-up report: Outcome summary on request
