Enable (NSW)
Enable NSW is a government agency in New South Wales, Australia. Its primary role is to provide support and services to people with disabilities, enabling them to live independently and participate in community life to the fullest extent possible.
Walk Easy works alongside Enable NSW to ensure that people with disabilities can live independently and actively participate in community life to the best of their ability.

Consumer
If you meet the criteria and have been assessed for Footwear by your Prescriber, we will require the following 2 documents to complete your application
Prescriber
If the consumer meets the criteria, we will require the following 2 documents to complete their application.

- “Equipment Request Form” (ERF) from you as the prescriber
- “Enable NSW Application Form” filled out by the consumer and any other information Enable NSW has requested.
