We will require a DVA “Footwear Prescription” and any other information you feel maybe helpful with the requested supply.
Appointments
Can be made at any of our clinic locations – Tamworth, Armidale, Gunnedah or Royal Prince Alfred Hospital – Diabetes Centre, by phoning our Tamworth Clinic on: 02) 67663153.
What the Veteran brings to the appointment?
Their footwear prescription
A pair of socks
The shoes that they will be leaving for modifications or repairs
The shoes that they will be leaving to have custom moulded orthoses fitted
Any other information that you think we might need
Their club membership or letter if they are applying for DVA recreational footwear
Home Visits
Only available at our Tamworth, Armidale or Gunnedah locations by phoning our Tamworth clinic on 02) 67663153.
Prior Approvals
We will require prior approval for the following supply requests:
Recreation Footwear, custom footwear, custom moulded orthoses and filler prosthesis, some modifications.
Approvals usually take approx. 3 weeks. Once we receive them, we can proceed with the request.
DVA ready-made Footwear Supply
Will be either sent to the prescriber, patient’s home or they may be required to come back for a fitting and collection.
Custom Footwear & Orthoses Supply
An appointment for fitting and collection is required, and we discuss the recommended guidelines for “wearing in new shoes and orthoses”.
A check-up appointment is arranged within 2-4 weeks following this “wearing in” period.
