If you are an Enable NSW Prescriber

We are recognised providers of Pedorthic Services with the Enable NSW Aids & Equipment Programme, which includes Medical Grade Footwear, Custom Cast Footwear, Custom Cast Footwear with internally built AFO’s, Custom Moulded Orthoses, Shoe Modifications and Repairs.

If the consumer meets the criteria, we will require the following 2 things to complete their application:

  1. “Equipment Request Form” (ERF) prescriber to fill out (downloadable here):
  2. “Enable NSW Application Form” filled out by consumer (downloadable here): along with any other information Enable NSW has requested.

Once both these forms have been completed and we have provided a quote the application will be ready to lodge with Enable NSW by email. enable@health.nsw.gov.au

Quote Appointments and Estimate Quotes

  1. For the Consumers convenience, Prescribers sometimes request an estimate quote for lodgement of application purposes. This means that we email an estimate quote to the prescriber based on the information they have provided to us by either emailing a description, clinical history, completed ERF and/or photographs
    This means that we won’t need to see the consumer for a quote appointment. We will however require a copy of the completed ERF for our records.
  2. Consumer quote appointments, whereby the consumer will be required to attend one of our clinic locations for the purposes of an assessment and quote.

Lodgement & Approval

Once we have provided an estimate quote or the consumer has been assessed and we have all the necessary paperwork to complete their application, we will attach our quote and lodge the application with Enable NSW.

Once we receive a purchase order from Enable NSW, we will contact the consumer to make an appointment to proceed with the requested supply.

Purchase Orders

New Footwear and orthoses
Once we have received the purchase order from Enable NSW, we can proceed with the footwear. We will contact the consumer to come back for measurements or to take plaster moulds/casts and the footwear will take approx. 4-6 weeks to complete.

Modifications/Repairs
Once we have received the purchase order from Enable NSW, we can proceed with the modification/repair request. We will contact the consumer to arrange an appointment, where they will be required to leave their footwear with us for modifications or repairs.

Footwear/Orthoses Supply

We will phone the consumer to organise their fitting and collection appointment.  This is where we do the fittings and make sure that the footwear/orthoses are fitting correctly and we discuss the recommended guidelines for “wearing in new shoes and orthoses”.

Check ups

It is a supply requirement that the consumer comes back for a check-up within 2-4 weeks following this “wearing in” period to make sure everything is working right for them.
