If you are a Consumer

We are recognised providers of Pedorthic Services with the Enable NSW Aids & Equipment Programme, which includes Medical Grade Footwear, Custom Cast Footwear, Custom Cast Footwear with internally built AFO’s, Custom Moulded Orthoses, Shoe Modifications and Repairs.

If you meet the criteria and have been assessed for Footwear by your Prescriber, we will require the following 2 things to complete your application:

  1. “Equipment Request Form” (ERF) from your prescriber (downloadable here):
  2. “Enable NSW Application Form” filled out by you (downloadable here):  along with any other information Enable NSW has requested.

Once you have both these forms completed, and require a quote appointment please follow these simple steps:

1. Quote Appointments

Can be made at any of our clinic locations – Tamworth, Armidale, Gunnedah or Royal Prince Alfred Hospital – Diabetes Centre, by phoning our Tamworth Clinic on:  02) 67663153.

2. What to bring to your appointment?

The Enable NSW “Equipment Request Form” (ERF) filled out by your Prescribing Podiatrist.
The Enable NSW “Application Form” that you are required to fill out
A copy of your Medicare card, pension card or ATO notice of assessment
A pair of socks
The shoes that you are leaving to be modified or repaired
The shoes that you are leaving to have your custom moulded orthoses fitted
Any other information you think we may need

Estimate Quotes

Prescribers sometimes request an estimate quote for lodgement purposes.
Sometimes we email estimate quotes to your prescriber based on the information they have provided to us. This means that we won’t need to see you for a quote appointment.

Once we receive a purchase order from Enable NSW, we will contact you to make an appointment to proceed with the requested supply.

Purchase Orders

New Footwear and orthoses
Once we have received your purchase order from Enable NSW, we can proceed with your footwear. We will contact you to come back for measurements or to take plaster moulds/casts and your footwear will take approx. 4-6 weeks to complete.

Modifications/Repairs
Once we have received your purchase order from Enable NSW, we can proceed with the modification/repair request. We will contact you to arrange an appointment, where
you will need to leave your footwear with us for modifications or repairs.

Check ups

It is a supply requirement that you come back for a check-up within 2-4 weeks following this “wearing in” period to make sure everything is working right for you.
